Why do certain monsters not respawn?

Boss monsters will not respawn again unless you leave the instance and reform your current party, resetting the instance. The group leader can also reset the instance with the "reset all instances" option, located by right-clicking the character portrait. All group members will need to exit the instance before it can be reset.
